Kids medical exemption affidavit help!!!

Hello all, could anyone please help me with a query regarding medicals and in particular excluding kids from having them! The reason I am requesting help is that I have read somewhere that if you supply an affidavit signed by a legal person, you can exclude your children from taking their meds.

I am unable to take may children to have medicals has my ex-wife will not allow them to go - therefore I would like to know if anyone knows what the contents of the affidavit should read????

Not got any personal experience but notice you are using an agent. They should be able to advise you on this and what you would need to do.
