Since you lodged in the first half of 2013, I highly recommend you to lodge a complaint with IGIS to check if your ESC has been completed or not. When were you contacted by Team 13 initially?

Yes, non-migrating dependent like Wife needs to supply medical examination and penal certificates. These requirements must come out positive for you to be granted a visa. Since 887 normally doesn't require medicals, you'll only need to submit penal certificates for your wife irrespective of where she is.
She came in October 2013 from Italy we got Pcc from 2013 but she never depart Australia , so do we need it ?
In my opinion since its been more than a year she is here she will need Australian PCC + Italy PCC if she has stayed in Italy for 12 months after turning 18 and also for any other country where she has stayed for 12 months cumulatively.
If you are in doubt you can directly contact your case officer. At the end of the day DIBP will ask for what they need, so just be mentally prepared and organize for the PCC once they ask you.
Also PCC's are generally valid for 12 months from their issue date, so anything issued in Oct 2013 is no longer valid unless DIBP extends it for you under the hood.
