Do we have to take our medical history and notes from our current doctors? If so, how do we do this? Will the surgery just give you copies of your notes?

We did. I think I paid 10 pounds each for copies. Handed them into new doctor - never know when they might be useful. Did the same for kids dentists as well.

The only medical records I bought were the kids injections as they have differernt ones here. I just told the doctor anything relevant.
