1221 Form - does it have to be filled and uploaded?

I'm uploading documents onto the online ImmiAccount and wondered if my wife needs to fill in the 1221 form? I ma the VISA applicant, however she will be travelling on my VISA. It doesn't say online that its required, however can anyone say from experience if they filled and uploaded this form? I'm assuming if we don't and we need to our CO when assigned will tell us whats outstanding?

What visa is it?......have you downloaded the appropriate required documents checklist? On there it will usually say who you need a particular thing for, ie " Completed form 80 for all applicants included on visa"

just looked at the form it says to be filled out by all applicants over 18, so if you require to fill one in for yourself then yes your wife will need one to
