Citizenship application

I am just in the process of completing our online citizenship application.

I have printed off a copy of the 1195 Identity declaration form to get completed. However i have been told by someone else there are 2 forms to be completed and uploaded.

Am I missing something? or has it changed from previously and it used to be 2 forms?

I dont want to mess it up by not loading the correct documents.

Although I never actually got around to putting my application in, I did do my ID forms and I only remember one.

the 1195 is a 2 page form. When you scan them in, you need to attach 2 pages. You also attach your photoid and a copy of the certified back of that photo I'd.
